Bridge21
B21 is a scattered community housing project that addresses the accessibility needs of individuals with Autism, Intellectual and Developmental Disabilities (A/I/DD) in Summit County, Utah.
The initial house opened in 2025, providing accessible housing for four neurodivergent individuals, and expand to three properties housing 12 individuals in 2027. B21’s key differentiator is addressing the affordable, cognitively accessible, long-term housing needs of our population while also providing the supportive amenities and services that will allow this population to live independently.

Stephanie Polukoff, OTR – Executive Director
Stephanie is a Pediatric Occupational Therapist with 30 years of experience running her own business, SAP Services, providing healthcare/therapeutic services to a variety of schools and clinics that support special needs children and their families.
She worked in the Park City School District for 17 years as well as Primary Children’s Hospital, Blue Sky Therapeutics, and private practice. The very *definition of her profession has prepared her for developing a co-housing inclusive community. She is a founding member of Bridge21 Park City and considers it her greatest honor to create an inclusive, sustaining, nurturing community for her special needs friends to thrive and succeed in life. She has also been an advocate for her own children growing up in Park City where she was President of school PTO’s, member of PCEF and PCSTF, Jans Winter Welcome volunteer and NAC volunteer when it was just one building and a barn. Currently, Stephanie is dedicated to B21, Walter, the pug, and her family, Jerry, Izaac and Natalya.

Occupational therapy (OT) is a global healthcare profession. It involves the use of assessment and intervention to develop, recover, or maintain the meaningful activities, or occupations, of individuals, groups, or communities.
Meredith Powell, CTRS – Program Coordinator
Meredith is the Program Coordinator at Bridge21 Park City.
As a Recreational Therapist with a background in addiction treatment and adaptive recreation, she is passionate about creating spaces that evoke belonging and connection, especially in diverse groups. With years of experience working with a wide range of populations, Meredith brings a wealth of knowledge in understanding the unique needs of different individuals. She believes that inclusion strengthens communities, and this philosophy is at the heart of her work at Bridge21.
A graduate of Clemson University, Meredith has called Utah home ever since. Outside of her professional work, you’ll find her playing outside, running, or listening to some good ol’ banjo music (or more likely, all three at once!). She also volunteers with YoungLife Capernaum in Park City, supporting local youth with disabilities.
Wes Stout – Board Chair
Mr. Stout is dynamic sales and operations executive with high-impact
success in engaging stakeholders at all levels through strategic
roadmaps, annual operating plans, market expansion, and deal
negotiations.
Over the course of his career, Wes has worked primarily in the
consumer packaged goods industry. He currently leads a sales team for
Keurig Dr Pepper and previously did the same at PepsiCo and Acosta Sales.
He also spent 3 years as a director with Young Life Park City where he started
Capernaum, Young Life’s neurodiverse outreach to students ages 12-21.
Mr. Stout graduated from Miami University of Ohio with degrees in
economics, geography, and urban planning. He is a founding board member
of Bridge21 and also serves on the Young Life Park City board.
Wes and his wife Hannah reside in Park City and both have siblings with
special needs. They are passionate about building community and hope Park
City can be a place where neurodiverse adults can have a thriving life.
Liza Howell – Board Member
Liza Howell is a founding member of Bridge21 Park City.
She is the mother of a young man on the autism spectrum and his 3 adoring sisters. An artist and children’s book author, Liza has volunteered countless hours to organizations she believes make a difference in the world.
Liza is president of the Kenan Family Foundation and co-director of the Hope through LIFE foundation with her husband Drew. Running, hiking, gardening, and leading Bible studies are some of her favorite activities.
Clay Weldon – Board Member
Clay has been with B21 from the very beginning.
My early years growing up back East outside Philly were spent living an adventurous life riding freestyle BMX, go-carts, unicycles, dirt bikes, playing all the sports, and… delivering lots of wedding cakes. My parents owned a bakery that’s still around today and I’m one of six kids who all worked in the bakery.
I have a beautiful wife, Cristy, and some amazing kiddos with one being a special needs son, Luke, who lights up everyone’s life.
I founded a fitness software business back in 2011, absolutely love what I do, and am surrounded by incredibly supportive people. Be content, but never settle.
Safia Keller – Board Member
Safia has been the Director of Development for the College of Education at the University of Utah since 2019.
Before this position, she was the Chief Employment Development Officer at Columbus Community Center for 3 ½ years, Director of Corporate Relations at Westminster College for 10 years from 2006-2016, and Director of Group and Corporate Ticket Sales for Broadway Across America for 11 years from 1995-2006.
She sits on several boards, including the AFP Utah Chapter, the University of Utah Marriott Library Ski Archives, the Rotary Club of Millcreek, CNS Home Health & Hospice, and MS Society Utah/So. Idaho Board Emeritus Board Member. She is also the past Chair of the Utah Women’s Giving Circle, which is part of the Community Foundation of Utah. She received her BA in International Relations at the University of Colorado in Boulder.
She has one son, Maclane, who is 27 years old and is the light of her life. She is a fierce advocate for the inclusion of people with disabilities in all aspects of life and making sure that DEI statements include disabilities as a form of diversity. When not working or volunteering, she loves to ski, go camping, and enjoy a nice glass of wine with a good book!
Joy Stanley – Board Member
Joy moved to Park City 23 years ago with her husband and 3 boys.
Her youngest son has been the family’s focus since his Autism diagnosis 24 years ago. Since then, Joy has managed an in-home program teaching him how to be a smart, fun, and kind adult while helping him grow to independence.
Originally from Massachusetts, Joy has a degree in Finance from VA Tech. She served on the board of the National Ability Center for six years and supports many neurodiverse causes locally and nationally. Traveling, hiking, skiing, and family gatherings are some of Joy’s favorites.
Amy Bailey – Board Member
A career marketing professional, Amy specializes in marketing strategy, event marketing and corporate communications.
Amy and her husband dreamed of living in a mountain town since their skis hit the snow in Telluride, CO many, many years ago. So in the Summer of 2020, they took a giant leap of faith and moved from Nashville, TN to Park City and have been blown away by all the resources available for their daughter, Brooke, who has special needs.
Amy earned a Bachelor’s degree in Broadcast Journalism and a Master’s degree in Journalism with a concentration in Public Relations. She spent 15 years with a top 3 telecommunications company where she managed all aspects of marketing including public relations, communication strategy, incentive programs and event management. She is currently the Client Relations Director for a Nashville-based healthcare compliance organization. Amy is also a graduate of Vanderbilt University’s Kennedy Center Volunteer Advocacy Project certifying her to advocate for families with special needs children in the Nashville school system.
When she’s not running Brooke between various NAC activities, Special Olympics events and her shift at Lucky Ones Coffee Shop, Amy can be found somewhere on a mountain hiking, trail running or skiing.
